ANALYSIS
Systematic studying of the causes or conditions that lead to a problem or enable it to persist.
RESPONSE
Implementing measures based on your analysis that appear likely to reduce or eliminate the problem.
ASSESSMENT
Evaluating to see if it worked as intended and whether the problem has been removed, reduced, or unintentionally aggravated.
